How do you know they're side quests until you play through at least once?

I feel like they often telegraph what you're getting into.
Many games tell you what the next main story objective is, while others make it require a little more inquiry.

I think Dragon Age: Origins did a great job of making you intrigued enough to bother, because there was progress tied to most things you did, and a sense of wonder was maintained.

Well yeah, side quests are a double-edged sword.
In my opinion, you either do them right or just leave them out all together.

I don't want to hear "Please kill 20 wolves for me okay bye".

I need some backstory. Some connection to the quest-giver. Some amount of insight as to why that person needs us to do something and what brought them into that situation.
