Active Status on Screen

Hello Guys, I need a script where your current status will display on screen like, the icon.
- When cured the Status Icon will disappear.
-(Optional) When hovered by mouse the description of that status will appear.
- every 120sec the name of the status will appear next to it then will fade and appear again.
