I would assume you would use a step count variable and a conditional branch(like stop healing when step count reaches x). I haven't made a step count variable yet though, so I can't really explain it to you.
What are the expectations in battle and out of battle? I've never heard of a HoT item. In battle, you would simply have the item heal you for x and then assign a HoT state which would act as a "reverse poison" for a set amount of turns.
Outside of battle... perhaps what just1witness says? Or perhaps make it a battle only item?
