IMHO, this might be a 13+ at least. I'ts quite artistic so I'd say its fine..

There are no nipples, or genitals so it isn't considered moderately sexual.

According to this, the beheaded bodies are...Fantasy Violence? Well, there's minimal blood (and I suppose there's a least some blood in your game) so "Teen" seems appropriate.

Sure, someone might disagree, but I highly doubt you could get in legal trouble with that picture.
