OK, so I am using SRD's Alt Menu Screen: Busts and Icons (http://sumrndm.site/ams-busts-icons/), but was wondering- what all do I need to change in the plugin's code to get the left side of the picture to look like the mockup on the right?

I have been messing with the code with my extremely minimal JS knowledge for this for a week now with little success.

Bumping this because after 3 days, I still can't figure out how to modify this plugin to look the way I want it to.

I haven't looked at MV code in a long time, but I'm going to hazard a guess that this should do the trick if you save it as a js file and add it to your project as a plugin below SRD's.
