It has become so common to see violence that I'm not even sure what falls into the "extreme violence" section anymore...
I'd say that killing someone should be extreme violence (as in, death is definitive - that's the extremity of pain) but it's really depending on the local culture.
I remember having someone commenting on my game, saying that a sex scene that happened was disturbing and unnecessary (even though they're 32px sprites and their bottom half is under the blanket). What amazed me is that the commenter didn't say a word about another scene where you slit throats of sleeping people.
I'd have thought that scene would have been disturbing...
I guess it's a cultural thing.

Do your game as you think it should be and don't bother about rating, unless you're aiming for a specific country.
